If you have misplaced or lost your UK birth certificate and require a replacement, you can order an official, certified copy online. In nearly all circumstances a full, long form certificate will be required which normally contains the following details:

- Name and surname of father
- Name, surname and maiden name of mother
- Occupation of father
- Signature, description and residence of informant
